This chapter introduces the field of online mental health, outlines its growing evidence base, and examines why implementation in routine practice has remained uneven despite clear potential. It discusses key advantages such as improved access, flexibility, reduced stigma, and cost-effectiveness, alongside major barriers including professional scepticism, concerns about effectiveness and relational quality, technological and ethical issues, and insufficient organisational and policy support. It also places these developments in the context of COVID-19 as a major catalyst for adoption, highlights variation across European countries, and points to the need for legal, regulatory, and reimbursement frameworks. Finally, the chapter situates this discussion within broader EFPA efforts to develop guidance for high-quality online mental health care.
